	OFFICE APPOINTMENTS General Information: Welcome to Gastroenterology Associates. We provide digestive and hepatic health services in the South Puget Sound region. Our main office is located in Olympia, with a satellite office in Shelton. Click here for directions. What To Bring To An Appointment: Bring a list of all medications you currently take, including over-the-counter and herbal supplements. Include the exact name of medication, dose of medication, and how often you take it. If it is more convenient, you may bring your medication in its original container. If you have any pertinent lab or x-ray reports that will assist in your evaluation, please ask your primary care provider to send those to us prior to your appointment. Bring your current insurance card(s) and photo identification. Co-payments and deductibles are due on same day that service is provided. In addition to your insurance, we accept cash, check, Visa, Discover and MasterCard. Insurance: We accept and bill for most insurance plans. When using insurance, please remember to verify, prior to your appointment, that we accept your insurance plan, that your plan is current, and that we have received a referral from your primary care provider, if required by your insurance. Fees: Fees are dependent upon each service provided. Laboratory, x-ray, and pathology services are billed separately. If you have questions about the costs involved, please contact our patient account representative at 413-8250. Additional fees may be charged for same day missed appointments. Cancel An Appointment: In the event you cannot keep your office appointment, contact our office at least 3 business days in advance.
